Enrollment

WELCOME TO EAST MUSKINGUM SCHOOLS

Registration for Residential Students (lives within district):

Our district has partnered with FinalForms, an online forms and data management service that replaces paper forms. FinalForms allows you to complete and sign K-12 student enrollment, back-to-school and athletic participation forms for your new or current student.

You can review and update your data at any time during the school year. You will be required to sign your online forms annually, and after any update you may make, such as a change of address or phone number. Registration must be completed by the parent or legal guardian.

For new students, the parent/guardian will need to provide the following documentation by either uploading copies in Final Forms, or bringing them to the school office so copies can be made:

  • Parent/Legal Guardian photo ID
  • Copy of child's birth certificate
  • Immunization records
  • Proof of residency- includes a deed, building permit, rental agreement, property tax statement, voter registration card, or completion of Form 5111 F2a or Form 5111 F2b (5111 forms are provided on our website under ‘Parents’-> ‘Forms’-> ‘Residency Affidavit and Verification forms’; these forms must be notarized). These are the only forms which are accepted as Proof of Residency. If the child resides with someone other than a parent, custody papers must be uploaded.
  • Current IEP/ETR/504 Plan (if applicable)

Registration for Open Enrollment Students (lives outside district):

For families living outside of the East Muskingum School District interested in enrolling their student, please follow the below instructions.

NEW Students: Please complete this form for new students wanting to open enroll to East Muskingum Schools - East Muskingum Local Schools Open Enrollment Application.  Do not complete the Final Forms process until you have been notified of your acceptance.

**Currently grades 1st through 8th are CLOSED to new open enrollment students for the 2026-27 school year. 

CURRENT Students: If your student has been accepted and are currently open enrolled in our district, you will need to re-apply each school year for open enrollment.  This is done through your FinalForms account.  Open Enrollment for the 2026-27 school year will begin on April 1, 2026.

Kindergarten Registration

For the 2026-27 school year, East Muskingum Schools registered students for kindergarten during the week of March 2 - March 6, 2026.

In accordance with Ohio House Bill 114, children who will be five years old on or before our first day of school (August 26, 2026), are eligible to register for kindergarten. 

If you missed Kindergarten Registration for the 2026-27 school year, please contact New Concord Elementary at 740-826-4453 OR Perry Elementary at 740-872-3436.

Preschool Enrollment

Preschool classes are located within the East Muskingum School District at New Concord and Perry Elementary Schools. These are operated by the Muskingum Valley Educational Service Center. To enroll your child or for more information, please contact the MVESC Preschool Programs or call 740-452-4518.

Homeschooling

Families living within our district that have decided to homeschool may submit their letter of intent by dropping it off at our District Office (13505 John Glenn School Road, New Concord, OH 43762), M-F from 8am-4pm, OR mailing it to the to the attention of the Superintendent at the same address. The letter of intent must be submitted annually.
